Rewards Program

So how does the Reward Program work?  It’s simple:

  1. Call Direct Path when you know you will have a procedure done.
  2. Direct Path will send you an email on cost comparisons.  You will have three options to choose from. 
  3. You choose the lowest cost provider to be eligible to receive a 20% up to $500.00 of shared savings when you select the ‘lowest’ cost provider.  
  4. Procedure must be performed within 180 days from date of the report it was emailed to you.
  5. Submit the EOB (explanation of benefits) along with the reward form to your Advocate.
  6. The Advocate will confirm that you selected the lowest cost option from the report and then HR will be notified.
  7. HR will receive a report monthly of eligible employees.
  8. Check will be issued separate from regular payroll check. (Just as FYI – it is taxed based on current deductions on your W4).

 

 

Remember – selecting the lowest cost not only saves you money, but also saves the University money, which helps keep premiums down year to year.  We pass the savings on to you.  The choice is up to you.
