Personnel Action Form (PAF)

Attached is the updated Personnel Action Form (PAF) to change an employee’s current status. When an employee has a change in position, this form will need to be filled out and submitted to the Human Resources Office after obtaining the required signatures. The new PAF will replace the offer letter that was previously sent to the employee.

As we update forms in Human Resources, we would like to thank you in advance for your patience. Please cease using all previous versions of the Personnel Action Forms (PAF).
